Getting ready for your visit

After you’ve booked your appointment,

• Fill out the Medical Forms prior to your scheduled visit
• Bring your ID and any insurance cards
• If you have health conditions that require your eyes to be dilated, please ensure you arrange for somebody to take you home safely
